Hypertension and ongoing monitoring and reviews
VENN PRIMARY CARE NETWORK ACTION ON HYPERTENSIONEffectively Monitor & Control your hypertension
People with a diagnosis of hypertension (high blood pressure) require regular monitoring. They should have their blood pressure checked at least annually. This is because our blood pressure is always changing and what was once well controlled blood pressure can change to become poorly controlled blood pressure and, as such, sometimes medication changes are needed. It is expected that once a person has a diagnosis of hypertension, they will require lifelong medication.
It is important that blood pressure is well controlled as persistently high blood pressure increases risk of strokes and heart attacks.
There are some lifestyle changes people can make to help with blood pressure, these include reducing salt intake, increasing activity levels, reducing weight and stopping smoking (if these are issues for you).
We know that in Hull there are many patients walking around with known hypertension whose blood pressure is higher than we would like and, as such, they are at a higher risk of having a stroke or heart attack. Please make sure you get your blood pressure checked when invited in by your practice.
